The submission of this Statement of Commitment finalizes my registration for a reserved spot at the Thomistic Institute’s Thomistic Philosophy and Natural Science Symposium. The TI is committed to stewardship and revoking my acceptance will cause significant financial difficulty. If I do not inform the TI of my cancellation by May 15, 2019, I agree to pay a $50 cancellation fee to help defray the cost incurred by the TI.
Furthermore, I hereby authorize the TI or its designee to photograph and/or record (including audio, video, etc.), and to later use for its non-profit purposes, my involvement in all aspects of this symposium. This authorization is granted with the understanding that the TI will assert a copyright, but not deprive me of any other rights I may have in the material presented. By submitting this form, I agree to release, waive, discharge, covenant not to sue, and hold the TI and its employees from and against any and all liabilities, demands, claims, or injuries that I may sustain during or in conjunction with the symposium.

Kindly email me at firstname.lastname@example.org if you have to cancel so that we can accommodate as many students as possible on the retreat. There is no penalty for cancelling before March 15th. If you have to cancel after March 15th, we’ll ask that you pay a $50 cancellation fee to help defray the cost incurred by the TI.
Also, I’ll be in touch in the weeks leading up to the retreat with final details.
